Plans Include:
Appointments
Thorough pre-session questionnaires to assess psychiatric history, medical history, lifestyle factors, and treatment goals
Medical records and recent labs reviewed as needed
Ordering and interpretation of basic lab work (done at Quest)
Prescriptions sent to the pharmacy of your choice
A basic supplement plan
1 straightforward or complex prior authorization
Timely communication
Message me directly through a secure portal
Schedule up to two 15-minute phone calls on an as-needed basis
A plan for ongoing care
Prioritize remaining symptoms and diagnoses
Under 18: a letter to your school explaining your diagnosis and medicines, and requesting accommodations (a 504 or IEP) if applicable
A letter transferring care back to your original prescriber or a long-term plan for follow-up at my office
Followup appointments in Family Plans can be shared between members of the same household.

First appointment within 2 weeks of your free introductory phone call if records of past diagnosis or treatment are available
Price varies with plan features
In my experience, clients who start with a structured care plan see more rapid improvement of symptoms. However, a-la-carte pricing is available for those who prefer it. Please schedule your free introductory phone call to discuss this option further.
