GREEN PLAN
Goals:
Assess for ADHD and rule out other causes of symptoms
Diagnose ADHD when appropriate
Start treatment
Achieve early symptom improvement
Determine the best option for ongoing support
Best for me if all of these are true:
I am between the ages of 5 and 60
I do not have any other medical or mental health conditions, or those conditions aren’t causing problems in my life right now
No one in my immediate blood family has had an early heart attack, heart rhythm problems, seizures, or bipolar disorder
I am not currently at risk of being expelled from school or demoted/fired at work
I am open to learning about stimulant medicines
I am at least 18 years old, or I am under 18 and my parents usually agree on the best treatment for me
This 90-day Plan Includes:
A 90-minute new client session
Thorough pre-session questionnaires to assess psychiatric history, medical history, lifestyle factors, and treatment goals
Medical records and recent labs reviewed as needed
Ordering and interpretation of basic lab work (done at Quest)
Four 25-minute follow-up medication management appointments within 90 days
Review of current medications & supplements for potential interactions and risk of vitamin or mineral deficiencies
Prescriptions sent to the pharmacy of your choice
A basic supplement plan
1 straightforward prior authorization
Up to two scheduled 15-minute phone calls
Clarify potential side effects
Make changes based on response to medication
Timely communication
Message me directly through a secure portal any time
Schedule up to two 15-minute phone calls on an as-needed basis
A plan for ongoing care
Under 18: a letter to your school explaining your diagnosis and medicines, and requesting accommodations (a 504 or IEP)
A letter transferring care back to your original prescriber or a long-term plan for follow-up at my office

In my experience, clients who start with a structured care plan see more rapid improvement of symptoms. However, a-la-carte pricing is available for those who prefer it. Please schedule your free introductory phone call to discuss this option further.
