Goals:

  1. Assess for ADHD and other possible causes of symptoms

  2. Prioritize treatment based on identified conditions

  3. Achieve early symptom improvement

  4. Determine the best option for ongoing treatment

Best for me if at least one of these statements is true:

  • I am younger than 5 or over 60 years of age

  • I have tried unsuccessfully to get diagnosed with ADHD in the past

  • I have tried several ADHD medicines and never found a good fit

  • I have another medical or mental health condition and symptoms cause difficulty in everyday life

  • I have a personal history of heart issues, seizures, bipolar spectrum, eating disorder, substance misuse, borderline personality, autoimmune conditions, or recent panic attacks

  • Someone in my immediate blood family has had an early heart attack, heart rhythm problems, seizures, or bipolar disorder

  • I am under 18, and my parents tend to have different opinions about my treatment

This 90-day Plan Includes:

  • A 120-minute new client session

    • Thorough pre-session questionnaires to assess psychiatric history, medical history, lifestyle factors, and treatment goals

    • Medical records and recent labs reviewed as needed

    • Ordering and interpretation of basic lab work (done at Quest)

  • Five follow-up medication management appointments within 90 days

    • Two 75-minute appointments and three 50-minute appointments

    • Review of current medications & supplements for potential interactions and risk of vitamin or mineral deficiencies

    • Prescriptions sent to the pharmacy of your choice

    • A basic supplement plan

    • 1 straightforward or complex prior authorization

  • Up to two scheduled 15-minute phone calls within 90 days

    • Clarify potential side effects

    • Make changes based on response to medication

  • Timely communication

    • Message me directly through a secure portal

    • Schedule up to two 15-minute phone calls on an as-needed basis

  • A plan for ongoing care

    • Prioritize remaining symptoms and diagnoses

    • Under 18: a letter to your school explaining your diagnosis and medicines, and requesting accommodations (a 504 or IEP) if applicable

    • A letter transferring care back to your original prescriber or a long-term plan for follow-up at my office
